Find a Doctor

Blue Card PPO® Network

Freelancers Insurance Company uses the Blue Card PPO® Network of doctors and hospitals. To find in-network providers near you:

1. Visit Anthem's site, select "Find a Doctor" from the toolbar on the right, and use the "Advanced Search."
2. Specify the doctor you're looking for (by specialty or name) and enter your location on the "Basic Information" tab.
3. Click on the "Insurance Plan Information" tab, select "Find my plan using my Alpha Prefix," and enter FGN.
4. Click "Search."

ZocDoc is a free service that allows patients to book appointments online and view user ratings of doctors. Note that only certain doctors in the Blue Card PPO® Network participate in ZocDoc, so your search won't include all of the in-network doctors in your area.

Find a Dentist

1. Visit www.guardiananytime.com/fpapp/FPWeb/dentalSearch.process
2. Choose your plan in the "Select a Plan" dropdown menu.
   • If you are on a PPO dental plan, select PPO.
   • If you are on an MDG dental plan, select DHMO/Prepaid.
3. Follow the prompts to complete your search.
