Long Term Disability :: Summary of Benefits
Below is a summary of benefits for Guardian's long-term disability insurance benefits. For more details view the complete benefits booklet .
Coverage Amounts
Members may choose one of the following flat benefit options, up to 50% of your current monthly salary: $500, $750, $1,000, $1,250, $1,500, $2,000, $2,500, $3,000.
Plan Features
Elimination Periods: An elimination period is the length of time between when a disability begins and receiving benefit payments from the insurer. Benefit payments for Guardian's long-term disability insurance will begin after you have been unable to work for a 30 or 90 day elimination period (depending on your election) due to disability.
Benefit Payments:For the first two years of disability, you will receive benefit payments while you are unable to work in your own occupation. After two years, you will continue to receive benefits if are deemed Activities Daily Living (ADL) disabled .
You will receive benefit payments to age 65, Standard ADEA while you remain disabled.
Guardian will waive your premium payment for this plan while you remain disabled.
Maternity: Maternity is covered as any other illness.
Conversion: If you choose to terminate your Freelancers Union membership, you can convert this policy to an individual policy (subject to the terms of your contract).
Limitations and Exclusions
Exclusions: Guardian limits the duration of payments for long-term disabilities caused by mental or emotional conditions, or alcohol or drug abuse. They do not pay benefits for charges relating to a covered person taking part in any war or act of war (including service in the armed forces); committing a felony or taking part in any riot or other civil disorder; or intentionally injuring themselves or attempting suicide while sane or insane. Guardian does not pay benefits during any period in which a covered person is confined to a correctional facility, a member is not under the care of a doctor, and the member's loss of earnings is not solely due to disability.
Pre-existing conditions: This long-term disability plan does not pay charges relating to a pre-existing condition. A pre-existing condition includes pregnancy and any condition for which an member, in the twelve-month period prior to coverage under this plan, consults with a physician, receives treatment, takes prescribed drugs, or exhibits symptoms which would cause an ordinarily prudent person to seek medical care or treatment. Please refer to plan documents for specific time periods.
Underwriting: All late entrants and increases in coverage amount require underwriting approval.
This policy provides disability income insurance only. It does not provide "basic hospital," "basic medical," or "major medical" insurance as defined by the New York State Insurance Department.
