3.a. Select Provider Type: All Physicians, Physician or Other Medical Professional or Hospital, Clinic, or Other Medical Facility
b. Select a Specialty Category
c. Click View Results

Search Tips:

• If you know your provider's name, you can click “Lookup By Name” in the right-hand corner of the webpage.
• If you're looking for a mental health provider, select "Behavioral Health" as the doctor’s specialty category. The specialty category is selected in Step 3b above.
