Important Enrollment Information
If you are enrolling in both Health and Dental Insurance, you must complete both enrollments at the same time. You will not be able to return to the enrollment site to enroll in an additional insurance product.
The deadline for enrollment in dental insurance is the 22nd of the month for coverage beginning on the 1st of the following month. If you enroll after the 20th of the month, you will not receive your insurance card on the 1st of the month. Your card should arrive by the 15th of the month. It will also take a few additional days for your enrollment to be effective in the Guardian system. This does not affect the start date of your dental coverage. If you need to see the dentist prior to receiving your dental card, you or your dentist can call Guardian Customer Service to verify your enrollment: PPO Customer Service: 800.541.7846.
Members may switch between MDG and PPO only during the open enrollment period (September of each year for an October 1 effective date).
If you cancel PPO coverage, and wish to re-enroll, you must wait for the open enrollment period that occurs a minimum of 12 months after the termination date. For example, if a member drops coverage in January 2009, the earliest date to re-enroll is October 2010.
